- Health Documentation - All students attending a Lakeway Christian School must have on file with the school office all medical reporting forms required by Tennessee state law. Families seeking exemption from state immunization requirements must complete and submit a notarized copy of the exemption documents for the state of Tennessee. Parents must provide the school with emergency numbers of friends and/or family who can pick up their sick child from school, if the child becomes ill.
