5.605 Medicines

    Distribution of Medicine - All prescription medication taken by students must be brought to school in the original, pharmacy-labeled container. The container shall display the following: 
    • Child’s name
    • Prescription number
    • Medication name and dosage
    • Administration route or other directions
    • Date Licensed prescriber’s name
    • Pharmacy name, address, phone number
     
    For all prescription and non-prescription drugs, a written request shall be obtained from the parent(s) or guardian requesting that medication be given during school hours. The request must include the parent’s or guardian’s name and phone number in case of emergency. It is the parent’s or guardian’s responsibility to ensure that the written request and medication are brought to the school. The parent or guardian must state that the child is competent to self-administer the medication with assistance.
     
    Upper School:
    For over-the-counter medication, a parent must bring the medicine in original container and annually complete an Over-the-Counter Medication Administration Form. 

    Lower School:
    There will be no over-the-counter medications supplied by or stored at the school for the students.
